Assertive Community Treatment

Our  Assertive Community Treatment (ACT) team is an interdisciplinary team of health professionals who provide community-based services to those with mental illness, who are often dually diagnosed with substance abuse.  ACT has often been described as a "hospital without walls."

ACT (formerly called the ROADS team) is primarily funded through a contract with the Missouri Department of Mental Health.  The ACT team collectively manages a caseload of 100 clients, with a 10:1 client to staff ratio.  The staff includes at least one psychiatrist, nurse, occupational therapist, employment specialist, and social worker.

In additional to mental illness, substance abuse addictions, homelessness, poor health, and unemployment are major factors that prevent client from accessing the care they need to live independently.  Clients see staff at least once a week in community-based settings, often the clients home.  Staff walk with clients, helping them learn to navigate and address a variety of issues, including housing, healthcare, and transportation, and employment.

Individualized treatment plans help clients achieve their personal goals while managing their symptoms, allowing them to maintain stable housing and live independently.  ACT is a national model for ending homeless, and research has shown astonishing levels of success.
